Coverage Highlights - Optional Term Life & Accidental
Death and Dismemberment (AD&D).
You will be asked to
acknowledge that you have read the Coverage Highlights
when completing the online enrollment form.
(2)
CHS Optional Term Life & AD&D Enrollment Form
One of the
required fields on the enrollment form is an employee ID
number. Anson Community Hospital Employees should
enter 000000 in this field. OR you may call AIA to
enroll.
This form will be
available beginning October 22nd at 12:01am through
midnight, November 12, 2008.
