Voluntary Benefit Options!
Voluntary Life Insurance and AD&D Coverage
In addition to the basic life insurance provided to you by
CHS, you may also purchase additional group term life
insurance for yourself or dependents as a way to supplement
your coverage. The enhanced options include the following:

Optional life coverage of 1, 2, 3, 4, or 5 times base salary
up to $1,000,000

CHS Open Enrollment dates. Click on the link on the

Part-Time Employees - Now you can choose Voluntary Term Life
and AD&D for yourself and your family!
Beginning January 1, 2009, employees with standard hours of
16 or more per week are eligible to purchase the
voluntary life insurance and
AD&D coverage described above
