UnumProvident is the provider for
this coverage. Short Term Disability is
only offered to Part Time employees who work 16 to
29.5 hours per week. This plan is non-occupational
coverage, meaning that on the job accidents and
sickness are not covered. Eligible employees can
purchase a benefit amount up to 60% of their monthly
salary ($400.00 up to $5,000.00). There are four benefit period options
available; 6 months, 12 months, 24 months., or 60
months.

POLICY OWNERSHIP AND PORTABILITY: UnumProvident’s Short Term Disability Insurance is
individual voluntary coverage and you are the owner
of your policy. Also, your policy is portable, which
means when you leave the company, you can take it
with you and still pay the same premium. UnumProvident
will bill you directly at home simply by calling our
office to request to be set up on direct bill at
home.
