When will I receive a policy?
The optional life insurance is self administered
by Carolinas HealthCare System along with the basic
term life insurance. Individual policies are not
issued. To download the Plan Certificate, click

Do I have to re enroll each year for this coverage
to continue?
No. As long as you remain eligible for this benefit (30 hours
per week) your coverage will continue.
How do a verify or change my beneficiary?
Forms are available through your benefits office.
